November is National Diabetes Month, and
a good time to re-visit some facts about this
chronic disease that affects so many of our
population.
Type 2 diabetes is the most common form
of diabetes, affecting millions of Americans,
and there are millions more people who are
not yet diagnosed.
When you eat food, the body breaks down
the sugars and starches into glucose, which Troy Caldwell, MD
is the basic fuel for the cells in the body.
Insulin, a hormone produced by our bodies,
is required for the body to use this glucose for energy as it takes
the sugar from the blood and transports it into the cells. In type 2
diabetes, either the body does not produce enough insulin or the
cells have become resistant to insulin. When glucose builds up
in the blood instead of going into cells, diabetes develops. Over
time, high blood glucose levels, or diabetes, may damage your
eyes, kidneys, nerves or heart. Cardiovascular disease is a major
complication of diabetes and the leading cause of death among
people with diabetes: about 65% of people with diabetes die
from heart disease and stroke.
Diabetes care focuses mainly on encouraging "self- management"
or educating each patient to manage their own disease, as they
are the person most affected by it.
The A-B-C approach to managing diabetes includes optimal con-
trol of A1 c, a lab test that gives the average blood sugar over the
past 3 months. Blood pressure control means that this reading
should be less than 130/80, and the"c" is for Cholesterol, which
should be controlled.to no more than 200 for the total cholesterol
reading, and no more than 100 for the LDL reading.
In general, every percentage point drop in Alc reduces the risk of
diabetes complications (eyes, nerves, feet, kidneys) by about 40%.
The Alc test should be done every 3-6 months for all diabetics.
Good blood pressure control reduces the risk of heart disease by
o
35-50, and should be checked every month or so. Improved
cholesterol control can reduce heart disease complications by 20-
50%. Cholesterol screening should be performed at least every
year or perhaps more often depending on medication response
and previous readings.
Dr. Coldwell will be speaking on diabetes and heart disease on
Thursday, November 8 at the NBHH Diabetes Support Group
monthly meeting. The meeting is held in the multipurpose
room at New Horizons Care Center.
